Student applies for a job and receives a job offer.

Student downloads document from HMSA website and prints it.

STATEMENT OF INTENT TO EMPLOY A MINOR AND REQUEST FOR A WORK PERMIT

Student emails HMSA Administration to schedule an appointment for a work permit.

Complete STATEMENT OF INTENT TO EMPLOY A MINOR AND REQUEST FOR A WORK PERMIT with employer’s and parent’s signatures is submitted to Administration prior to the scheduled appointment to process the work permit.

At scheduled appointment, Administration and the student sign work permit documents, review student expectations, and notify student about expiration date.

To renew work permit, student must complete steps 2-5 a week prior to expiration date.
