Mission Statement

Our mission is to create and foster an environment that provides opportunities for student-athletes to enrich their high school experience through participation on athletics teams. HMSA is dedicated to providing opportunities for all students to enhance the intellectual, social, physical, moral, and cultural development of the whole student/athlete; while conducting all activities with honesty and integrity in accordance with the principles of good
sportsmanship and ethical conduct.

Parents and Students:

Every student who wishes to participate in extracurricular sports must be cleared with the Hawthorne Math & Science (HMSA) athletic office before participating in tryouts, practice or games. Students will be cleared for sports (within 48 hours) once the following requirements are met & turned in to the HMSA front office or the athletic director.
 

1. Physical Examination- The student must be examined by a physician, who must fill out, sign, and date a physical examination form. The form is contained within the physical packet or the use of one provided by a physician is acceptable. Physical examinations are only accepted for a 1 year
period.

2. Physical Packet – This packet must be filled out completely and signed by a parent / guardian and student participant where noted. Physical packet includes

a. Physical evaluation form
b. CIF code of ethics
c. High School athletic Rules/ information
d. Program expectations
e. Athletic Insurance Certification Form
f. Medical treatment authorization waiver
g. Parental permission, assumption of risk, and medical treatment authorization
h. CIF concussion information sheet / Concussion symptom checklist
i. Sudden Cardiac Information Sheet
j. Authorization for photographing and electronic recording
k. Fundraising agreement form
l. Emergency information form

 3. PLEASE NOTE - State Law requires that any student wishing to participate in interscholastic athletics must have adequate medical insurance before being allowed to participate. The Athletic Insurance Certificate, contained within the physical packet, specifies the minimum requirements; if the student is covered by valid insurance which meets those requirements, the Certificate must be filled out completely and signed by the parent. For students not covered by acceptable insurance, a low cost medical and dental accident plan administered by Myers-Stevens & Toohey Company, Inc for students can be purchased.

When all forms have been filled out completely and signed, the student should take them to the HMSA front office or athletic director. Athletic Clearance will be processed within 48 hours. If all requirements have been met, the student will be cleared to try out, practice, and play games.

Competitive Athletics EC 221.9-

Pursuant to Education Code section 221.9, and beginning in the 2017-2018 school year and every year thereafter, Hawthorne School District is required to publicly report information regarding its competitive athletics to include total enrollment at Hawthorne School District, classified by gender; the number of pupils enrolled at Hawthorne School District who participate in competitive athletics, classified by gender; and the number of boys' and girls' teams, classified by sport. Please note that the data below reflects the total number of players on a team roster on the official first day of competition.

The district's athletic program shall be free from discrimination and discriminatory practices prohibited by state and federal law, including, but not limited to, the use of any racially derogatory or discriminatory school or athletic team name, mascot, or nickname. The Superintendent or designee shall ensure that equivalent athletic opportunities are provided for males and females, and that students are permitted to participate in athletic activities consistent with their gender identity.

Any complaint alleging discrimination in the district's athletic program shall be filed in accordance with the district's uniform complaint procedures.
